How to Make the Most of Your Office Visit:

• Arrive 10-20 minutes prior to your appointment time
• Bring your current insurance card(s)
• Make sure your address, phone numbers, email address and insurance information in your chart is correct
• Bring your medications or a current list
• Bring hospital/ER discharge paperwork or directions from other providers to your appointment
